Coffee, Polyphenols and Breast Cancer: What an Important New Study Found

Coffee polyphenols and breast cancer are the focus of a fascinating new study that found higher intake of certain plant compounds was associated with lower mortality and recurrence among women diagnosed with breast cancer. Even more interesting for coffee drinkers, coffee was the largest dietary source of those polyphenols in the study.

Coffee has a funny way of continually surprising us.

For centuries, we drank it because it tasted good, helped us wake up and gave us an excuse to sit down with another human being for twenty minutes.

Then scientists started taking a much closer look at what is actually inside the cup.

And a new study published in the British Journal of Nutrition has uncovered an intriguing association involving polyphenols — naturally occurring plant compounds found abundantly in coffee — and outcomes among women diagnosed with breast cancer. (⁠PubMed)

The findings deserve attention.

They also deserve a little perspective.

First, What Are Polyphenols?

Coffee isn’t simply caffeine dissolved in brown water.

Roasted coffee contains hundreds of biologically active compounds, including a large family of plant compounds called polyphenols. Among them are phenolic acids, including the chlorogenic acids for which coffee is particularly well known, along with compounds classified as lignans. (⁠American Cancer Society)

Polyphenols are also found throughout the plant world — in fruits, vegetables, whole grains, seeds, tea and other foods.

Researchers have become increasingly interested in these compounds because of their potential antioxidant, anti-inflammatory and other biological activity.

And that’s where this new breast cancer study becomes particularly interesting.

What Did the Researchers Find?

Researchers from the Federal University of Santa Catarina followed 95 women diagnosed with breast cancer for an average of 11.5 years.

Using dietary questionnaires and the Phenol-Explorer database, they estimated the women’s consumption of different classes of polyphenols and compared those intakes with breast cancer recurrence, survival and mortality. (⁠PubMed)

Women with higher consumption of several types of polyphenols experienced significantly different outcomes.

Higher total polyphenol intake was associated with a lower risk of breast cancer-specific mortality.

Higher phenolic acid intake was associated with lower breast cancer-specific mortality, lower all-cause mortality and a lower incidence of breast cancer recurrence.

Higher lignan intake was also associated with lower breast cancer-specific mortality. (⁠PubMed)

And then comes the sentence that caught my attention:

Coffee was the major contributor to total polyphenol, phenolic acid and lignan intake among the women in the study. (⁠PubMed)

That’s remarkable.

But it doesn’t mean what a sensational headline might lead you to believe.

No, This Doesn’t Mean “Coffee Cures Breast Cancer”

Let’s get that out of the way immediately.

This study does not demonstrate that drinking coffee prevents breast cancer, treats breast cancer or causes someone to survive breast cancer.

It was an observational study involving only 95 women. Researchers observed an association between dietary polyphenol intake and outcomes; they did not randomly assign patients to drink coffee or consume specific amounts of polyphenols.

In fact, the study’s authors specifically say that additional research is needed to confirm their findings. (⁠PubMed)

There’s another important distinction.

Researchers measured polyphenol consumption, not “coffee treatment.”

Coffee happened to be the largest dietary source of those compounds in this particular population.

That makes coffee an especially interesting piece of the story — but not proof that coffee itself produced the results.

What Makes Coffee So Interesting?

Coffee is one of the most significant dietary sources of polyphenols for many regular coffee drinkers.

One important family is chlorogenic acids, naturally occurring compounds in coffee beans that have been extensively studied for antioxidant, metabolic and anti-inflammatory activity. Scientists are also investigating potential mechanisms through which chlorogenic acids and related phenolic compounds might affect cancer biology, although much of that mechanistic research remains laboratory or preclinical work. So instead of thinking about coffee exclusively as a caffeine-delivery system, it’s more accurate to think of brewed coffee as a remarkably complicated plant beverage.

Hundreds of compounds begin in a coffee seed, are transformed by roasting, extracted by brewing and eventually end up in your cup.

We’re still learning exactly what many of them do inside the human body.

What Does Previous Breast Cancer Research Say?

Here’s where things get even more interesting — and more complicated.

A much larger 2015 European study followed 11,782 women with breast cancer and examined polyphenol intake before diagnosis. Researchers found an association between higher lignan intake and lower breast cancer mortality among postmenopausal women, but they did not find the same relationship for most other polyphenol categories. The results also differed among premenopausal women, illustrating just how complex these relationships may be. (⁠PubMed)

And an earlier Swedish study involving 3,243 women with invasive breast cancer found no association between coffee consumption before diagnosis and either breast cancer-specific mortality or overall mortality. (⁠PubMed)

In other words:

Science isn’t telling us, “Drink X cups of coffee and your breast cancer risk drops by Y percent.”

We’re nowhere near being able to say that.

What we’re seeing instead is an evolving body of evidence suggesting that the plant compounds found in coffee — particularly polyphenols — deserve serious scientific attention.

Coffee and Cancer More Broadly

Coffee’s relationship with cancer has been studied for decades.

The International Agency for Research on Cancer has concluded that available evidence suggests a lack of carcinogenicity for coffee drinking in relation to female breast cancer, among several other cancer types. (⁠IARC)

The American Cancer Society likewise notes that roasted coffee contains hundreds of biologically active substances, including flavonoids, lignans and other polyphenols, and that research has identified possible protective associations between coffee and several cancers. (⁠American Cancer Society)

The evidence is considerably stronger for certain cancers — particularly liver and endometrial cancer — than it currently is for breast cancer. (⁠World Cancer Research Fund)

And that’s an important distinction.

The Bigger Story May Not Be Coffee Alone

Perhaps the most useful way to read this new research isn’t:

“Coffee protects against breast cancer.”

It’s:

“Plant-rich diets containing higher levels of polyphenols may have important biological effects that we’re still trying to understand — and coffee happens to be one of our most significant dietary sources of those compounds.”

That is both more scientifically accurate and, frankly, more fascinating.

A coffee bean isn’t manufactured in a laboratory.

It’s the seed of a fruit.

And inside that seed is an enormously complicated collection of acids, carbohydrates, lipids, aromatic precursors and plant chemicals that interact with heat during roasting and water during brewing.

Every time science takes a closer look at coffee, we learn that there’s a lot more happening in that cup than caffeine.

So Should You Drink More Coffee?

Not because of this study.

Someone currently undergoing treatment for breast cancer shouldn’t change their coffee consumption, diet, medication or treatment based on a coffee-shop blog — or based on one observational study.

Those decisions belong in conversations with physicians, oncologists and registered dietitians who understand the individual’s treatment and medical history.

But if you already enjoy coffee, this research gives us another fascinating reason to appreciate what an extraordinarily complex natural product it is.

Coffee delivers aroma.

Coffee delivers caffeine.

Coffee brings people together.

And apparently, it’s also delivering a substantial portion of the polyphenols researchers are now investigating for their relationship with long-term health.

We’ll gladly raise a mug to continued research.

Because the deeper science digs into coffee, the more interesting that simple cup in front of us becomes.
